Enrollment Questions:

 HOW DO I ADD A NEW EMPLOYEE?

Complete an Application/Change form for Group coverage and fax or mail it to our office.  These forms are located on our website, or contact our office and we will send one to you.

 HOW DO I REMOVE AN EMPLOYEE FROM MY MEDICAL PLAN?

We need all terminations in writing.  You can email us at main@centbucks.com the termination request and include the name of the employee and their last date of employment.  The employee will be termed the last day of the month they worked, example:  If their last date of employment was July 2, 2015, they will be removed effective July 31, 2015.

 WHEN SHOULD I ADD MY NEWBORN?

Newborns must be added within the first 30 days of birth.  Complete an Application/Change form and fax or mail it to our office.

 HOW LONG ARE MY CHILDREN ELIGIBLE TO STAY ON MY COVERAGE?

Per Affordable Care Act guidelines, adult children may remain on your plan up to the age of 26.

 HOW DO I ADD / REMOVE A DEPENDENT?

Complete an Application/Change form for Group coverage and fax or mail it to our office.  Check the appropriate box next to add or delete a Dependent.  Complete the dependent information and sign the form. These forms are located on our website, or contact our office and we will send one to you.

 HOW DO I CHANGE MY PRIMARY CARE PHYSICIAN WITH KEYSTONE?

You can contact Member Services at 1-800-ASK-BLUE (1-800-275-2583) and give them your new primary care physician information over the phone.  OR, you can go online at www.ibxpress.com, register your account, and click on the “Change my Primary Care Physician” link. Or, you can email us at main@centbucks.com with the new Primary Care Physician information (name and Office ID #, NOT the phone number) and we will do it for you.

 IS THERE A REIMBURSEMENT FOR FITNESS CENTERS?

With Independence Blue Cross you can receive up to $150 when you complete 120 visits at an approved facility, within a year.  No more books to fill out; just go online at www.ibxpress.com and keep track of your workouts there.

 IS THERE A REIMBURSEMENT FOR WEIGHT MANAGEMENT?
